How does XEHR pricing work?
XEHR uses two pricing models depending on your facility type:
- Outpatient practices (physician, dental, urgent care, behavioral health, PT, ASC, FQHC, multi-specialty) → per-provider per-month module tier pricing.
- Enterprise facilities (hospital, SNF, home health, dialysis, pharmacy, lab, imaging) → flat per-facility per-month pricing, often with usage add-ons (e.g. per-OR, per-prescription, per-requisition).

What are the outpatient module tiers and what do they cost?
Three tiers, all priced per provider per month. Staff users (front desk, billers, nurses, admins) are unlimited and cost nothing extra.
- Clinical — $189/provider/mo ($1,500 setup): charting, scheduling, patient registry and e-prescribing. No claims or billing.
- Practice Suite — $449/provider/mo ($2,500 setup) — most popular: everything in Clinical plus full billing: claims, automatic payment posting, eligibility checks, denial management, the patient portal and quality reporting.
- RCM as a Service — 2.5% of monthly collections ($2,500 one-time setup; EHR, Practice Management & RCM all included): we handle billing end-to-end with a dedicated billing team.
How is enterprise facility pricing structured?
Flat per-facility (or per-bed for hospitals) per-month rates:
- Hospital: $7,999/mo (1-50 beds), $19,999/mo (51-200 beds), $49,999+/mo (200+ beds). Setup $25K-$75K.
- ASC (Surgery Center): $1,499/mo + $399/OR/mo. Setup $7,500.
- SNF / Long-Term Care: $1,999/facility/mo. Setup $10,000.
- Home Health: $1,499/agency/mo. Setup $7,500.
- Dialysis Center: $1,999/facility/mo. Setup $10,000.
- Pharmacy: $299/location/mo + $0.25/prescription. Setup $3,500.
- Clinical Laboratory: $499/lab/mo + $0.15/requisition. Setup $5,000.
- Imaging Center: $799/center/mo + $199/modality/mo. Setup $5,000.
All facility tiers include private, separate data for your organization, a signed BAA, and business-hours support with 24/7 emergency escalation.
How is AI usage billed?
AI is pay as you go, with no base fee: $25 per 100,000 tokens (a token is roughly a word processed), or discounted bundles of 500K tokens for $99 and 2M for $349. Usage is tracked per organization, and you can see it at any time. See the pricing page.
Do you require an annual contract?
No. XEHR is month-to-month. You can cancel any time with 30 days notice, and we export your data in standard formats at no charge.
Are there hidden fees for clearinghouse, e-prescribing, or patient portal?
The major modules — clearinghouse claim submission, electronic ERA posting, e-prescribing (including EPCS), patient portal, AI-assisted documentation, MIPS reporting, and the signed BAA — are included in the Practice Suite tier. The built-in clearinghouse and the AI are billed per use, at the rates on the pricing page; there are no other per-claim or per-prescription fees from XEHR.
Note: EPCS identity-proofing for controlled-substance prescribing may carry a small fee from the identity-proofing vendor, paid to them directly.
Can I mix or upgrade module tiers later?
Yes — you can upgrade between Clinical → Practice Suite → RCM-as-a-Service at any time with no data migration (everything stays in your same dedicated instance). Some practices also mix tiers across providers (e.g. Clinical for charting-only providers, Practice Suite for billing providers) — talk to sales to scope.
Is there a free trial?
