What makes critical care medicine billing hard

01

Time is the service

Critical care is paid for documented time, minus time spent on separate procedures.

02

Several doctors a day

Day and night intensivists each document and bill their own time.

03

Procedures at the bedside

Lines, intubations and bronchoscopies are billed separately when documented.

How critical care medicine care is delivered and billed

Intensivists trained in anesthesiology, internal medicine, surgery or emergency medicine work in medical, surgical, cardiac and neurological intensive care units, and increasingly provide remote tele-ICU coverage. All work is billed on the professional claim with the hospital place of service, mostly as critical care time and bedside procedures.

What payers check on critical care medicine claims

  • Critical care is billed by total documented time on the date, excluding separately billed procedures.
  • Some procedures are bundled into critical care and cannot be billed separately.
  • Only the time the physician spent on the patient’s care counts, not time of others.
  • Sepsis with septic shock is coded with the infection first.
  • When the patient is no longer critical, care is billed as hospital visits.

R65.21Severe sepsis with septic shock
J80Acute respiratory distress syndrome
I46.9Cardiac arrest, cause unspecified
N17.9Acute kidney failure, unspecified
G93.41Metabolic encephalopathy

Examples only. The provider chooses the code that matches the documented diagnosis, and XEHR checks it against the note and the payer's rules before the claim goes out.
