What makes hospitalist billing hard

01

Missed daily charges

A visit seen but not entered is lost revenue, and it happens every shift.

02

Observation or inpatient

The patient status decides how the visit is billed.

03

Several hospitalists, one patient

Group members seeing the same patient must not bill the same service twice.

How hospitalist care is delivered and billed

Hospitalists care for patients admitted to the hospital, from admission to discharge, and often cover observation units and consults. Everything is billed on the professional claim with the hospital place of service: admission, daily visits, discharge and observation services, each with its own rules. Many groups work with nurse practitioners and physician assistants, which brings split and shared visit rules.

What payers check on hospitalist claims

  • Hospital visit levels are chosen by medical decision making or total time on the date.
  • Discharge day management is billed by time, and more than 30 minutes must be documented to bill the higher level.
  • Inpatient versus observation status follows the payer’s rules, including Medicare’s two-midnight expectation.
  • Split or shared visits are billed by the practitioner who performed the substantive portion, as documented.
  • Only one hospital visit per day per specialty group is usually paid.

Examples only. The provider chooses the code that matches the documented diagnosis, and XEHR checks it against the note and the payer's rules before the claim goes out.

A hospitalist visit, start to payment

  1. 1

    Admit

The admission note records history, exam and the plan, with inpatient or observation status clear.

  2. 2

    Round daily

Each day’s note records progress and decisions, with the level supported by decision making or time.

  3. 3

    Share the work

When an NP or PA shares a visit, who did the substantive part is documented.

  4. 4

    Discharge and bill

Discharge time is recorded and every day goes out on a checked claim.
