Pathology Practice Software and Billing

Pathologists rarely see the patient, but their claims depend on details from the referring provider. XEHR keeps orders, reports and claims together.

What makes pathology billing hard

01

Missing information from the referrer

Claims fail when the ordering provider, diagnosis or insurance details are incomplete.

02

Professional and technical parts

Lab work is split between the reading and the processing, billed by different parties.

03

Many units per specimen

Several specimens from one case must be counted and billed correctly.

How pathology care is delivered and billed

Pathologists work in hospital laboratories, independent labs and practices. Anatomic pathology examines tissue and cytology specimens; clinical pathology oversees laboratory testing. Billing is professional, technical or global depending on who prepared the slides and who interpreted them, and each specimen is billed separately. Clinical laboratories must hold the right federal certification for the tests they perform.

What payers check on pathology claims

  • Surgical pathology is billed per specimen, so separately submitted and examined specimens must be listed separately.
  • Professional and technical parts are billed separately when different entities prepare and interpret the slides.
  • Special stains are billed per stain and per specimen when documented as needed.
  • Medicare’s date-of-service rule decides whether some tests on specimens taken during a hospital stay are billed by the lab or the hospital.
  • Laboratories may bill only for tests their federal certification covers.
Common pathology diagnoses ICD-10-CM
C44.91Basal cell carcinoma of skin, unspecified
D12.6Benign neoplasm of colon, unspecified
R87.610Atypical squamous cells of undetermined significance on cytologic smear of cervix (ASC-US)
K29.70Gastritis, unspecified, without bleeding
C50.911Malignant neoplasm of unspecified site of right female breast
N87.1Moderate cervical dysplasia

Examples only. The provider chooses the code that matches the documented diagnosis, and XEHR checks it against the note and the payer's rules before the claim goes out.
