What makes cytopathology billing hard

01

Adequacy checks at the bedside

Immediate evaluation during a biopsy is billed per episode, and each episode must be documented.

02

Screening or diagnostic Pap tests

Screening and diagnostic cervical cytology follow different coverage and frequency rules.

03

Split technical and professional work

Who prepared the slides and who read them decides which part each party bills.

How cytopathology care is delivered and billed

Cytopathologists work in hospital and reference laboratories and attend biopsy procedures for on-site evaluation. Interpretations are billed as professional services; slide preparation is billed by the lab. Gynecologic cytology follows screening rules set by Medicare and other payers.

What payers check on cytopathology claims

  • On-site adequacy evaluation is billed per episode, with each episode documented.
  • Screening Pap tests have frequency limits under Medicare.
  • Professional and technical parts are billed separately when different entities perform them.
  • Abnormal cervical cytology findings have their own codes.
  • Laboratories may bill only for tests their certification covers.
Common cytopathology diagnoses ICD-10-CM
E04.1Nontoxic single thyroid nodule
R87.610Atypical squamous cells of undetermined significance on cytologic smear of cervix (ASC-US)
R87.612Low grade squamous intraepithelial lesion on cytologic smear of cervix (LGSIL)
R59.0Localized enlarged lymph nodes
R91.1Solitary pulmonary nodule
Z12.4Encounter for screening for malignant neoplasm of cervix

Examples only. The provider chooses the code that matches the documented diagnosis, and XEHR checks it against the note and the payer's rules before the claim goes out.
