Authorization for imaging and surgery
Advanced imaging and spine procedures usually need approval with conservative treatment history attached.
Neurosurgical claims are complex and high-value, and payers review them closely. XEHR keeps imaging, authorizations, operative notes and claims connected.
Low back pain radiating down the left leg.
M51.16 Lumbar disc disorder with radiculopathy
Advanced imaging and spine procedures usually need approval with conservative treatment history attached.
Each level and procedure must be documented and billed in the right order, or payment drops.
Major procedures carry long follow-up periods, with separate rules for unrelated care.
Neurosurgeons see patients in clinic and operate in the hospital, with a heavy share of emergency and trauma work. Spine surgery, cranial surgery and procedures for pain are billed by the surgeon with the hospital or surgery center place of service. Long global periods, frequent prior authorization for spine procedures and complex multi-level operations make the documentation unusually consequential.

Examples only. The provider chooses the code that matches the documented diagnosis, and XEHR checks it against the note and the payer's rules before the claim goes out.
The consultation records symptoms, exam, imaging and earlier treatment, which payers require before approving spine surgery.
The case is booked at the hospital and the prior authorization is prepared with the imaging and conservative care history.
The operative note records each level, approach and implant, so the claim reflects the whole procedure.
The claim goes out with the facility place of service; follow-up inside the 90-day global is tracked.
