F03.90 Unspecified dementia, unspecified severity
Older adults are seen in clinic, at home, in assisted living and in nursing facilities. Each visit must carry the place of service where the patient was, or the claim is denied.
Memory loss, low mood and confusion can look alike and often occur together. The note has to separate them and code dementia with its severity and any behavioral disturbance.
Families and facility staff carry out the plan. They need clear instructions, and their involvement has to be documented with the patient’s consent or legal authority.
Geriatric psychiatrists work in clinics, hospitals, nursing facilities and patients’ homes. Visits are billed on the professional claim, mostly to Medicare, with the place of service set by where the patient was seen.
